SSHRC Form 1 Payment Activation Form PART II
SSHRC recipients must fill out SSHRC FORM 1, PART II and submit it to Grad Studies. Grad Studies will fill out the program confirmation section of this form and email it to SSHRC with a cc to student.
Forms must be signed and can be submitted:
- by email as scanned PDF attachment to appropriate Awards Clerk
- by fax to 604-822-5802
- by mail or in person to 170 – 6371 Crescent Road, Vancouver, BC V6T 1Z2
Important notes
- students need not fill out Part I of this form, since they must complete the External Graduate Award Activation Form.
